- Site and asset identifiers, process duty and whether standby capacity is available.
- The symptom and the relevant operating condition, including recent process or maintenance changes.
- Existing readings or reports, with dates and measurement conditions where known.
- The shutdown or operating window, site contact, permit arrangements and any equipment or access restrictions.
